Sports Medical Forms Submission Process
Submitting Medical Forms to an Athletic Staff Member does not clear
your child to participate in any sport.
IF YOU TURNED IN YOUR FORMS FOR SPORTS, IT IS IN PROCESS, SEE BELOW.
GENESIS WILL STATE THAT YOU ARE MISSING FORMS UNTIL THE PROCESS IS FINISHED.
State Mandated Guidelines:
- Sports Medical Forms must be completed, including the current physical https://mhs.montclair.k12.nj.us/athletics/mandatory_athletic_forms
- Current Physical date is entered into Genesis by a Certified Athletic Trainer
- School nurse reviews and any missing information is relayed to the parent
- School physician signs off on Medical form
- Medical form is returned to the School Nurse
- A School Nurse will examine whether the School Physician approved and cleared the athlete
- A School Nurse will enter the cleared athletes in Genesis
- A Certified Athletic Trainer will notify coaches of completion
- This process could take up to 3 weeks, depending upon the availability of School Nurse and the School Physician
- A report is generated of cleared athletes by a Certified Athletic Trainer and is emailed to coaches
