Hello Mountie Athletic Community,

I would like to announce that we will be using Genesis for all sports forms for the upcoming spring season.   You can register from February 8th to March 12th.

We have opened a new Tab in your Parent and Student Access accounts called “Athletics”.

This screen will display the date of your last physical exam, and when it expires.

It will also list all of the dates you signed your required forms.

All Genesis forms must be completed no later than March 12th at midnight.

All Sports Physicals must be handed in no later than March 12th.

Please follow the steps below in order to sign up for Spring Sports. Please log onto your Parent Access account and go to the Forms Tab.

  • Please remember: DO NOT share your password with your child and allow them to log onto your account.
  • Click on the form called “Spring Sports 2021”.
    • Please review the first 4 questions on the form.
      • Click on the link and review the document displayed.
      • After reading the document click on the “Selected” check box to the right of the question for the following forms.
        • NJSIAA’s Steroid testing policy.
        • Sports-Related Concussion and Head Injury Fact Sheet.
        • Sudden Cardiac Death Pamphlet.
        • Opioid Video Sign Off Sheet
        • Opioid Use and Misuse educational fact sheet.
    • Review the NJSIAA Steroid testing Policy, and choose yes to consent to random testing.
    • Read the districts Athletic Code of Conduct, and choose “Yes” that you and your child will abide by its provisions.
    • Choose the sport your child wants to play from the dropdown menu.
    • Click the green box called “Update Answers”.
    • Click the Finalize check box.
    • Click Ok when asked are you sure you want to finalize this form?
    • Click Update Answers.
    • If you do not answer all of the questions on the form, the form will not be accepted and your child will not be signed up for the sport they wish to play.

All medical forms are located on the MHS High School website under Athletics, Mandatory Forms.


You must register every athletic season (i.e. Fall, Winter, and Spring)

You will still have to hand in the full medical packet (4 forms). You must have a valid physical on file in the Nurse’s office in order to play sports.


Athletes can email completed physicals from their Primary Doctor and or the COVID Health History Updated Questionnaire to Ms. Kim Covello at


Please note that if you do not fill out all of your forms correctly, this could delay your participation.


March 12th will be the last day to turn in Sports Forms.

Physicals – There will be no further extensions for the 2020-21 school year, therefore the DOE regulations regarding physicals will be the rule. 
·       All student-athletes must have an approved physical before they are permitted to begin any athletic season.  Sports physicals are good for 365 days of exam.  
·       If a physical expires during a particular sport, the student-athlete may continue until that sport is completed but must get a new physical before their next sport.  Please be advised that some school districts may require you to get a physical before that 365 time period ends.  
o   CL1: A basketball players physical that expires on Dec 1, 2020 will need a new physical to start the season on Jan 11, 2021. 
o   CL2: A basketball players physical that expires on Feb 1st, 2021 can continue to the end of the basketball season unless the school requires a new physical by Feb 1st to continue. 
o   CL3: A basketball players physical expires on Jan 25th, 2021, and wants to swim, that student-athlete will need a new physical by the start of swimming season on Feb 1st.  
o   A football player’s physical that expires on July 15th, 2021 and wants to also compete in basketball, winter Track, wrestling and baseball is permitted for all 5 sports.
Health History Update Questionnaire – There are no changes to this DOE requirement.  Any athlete that took the extension this past fall was required to complete the form.  Going forward the DOE requirement for the HHQ form is the following: 
To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian
