Note: These physicals are intended for those who do not have a medical home or cannot get a physical with their personal doctor.

Medical Forms

The forms below must be printed, completed, and submitted to the School Nurse.
  • Forms should be placed in an envelope which is then sealed
  • Write the Student's name and sport clearly on the envelope
  • Freshmen should submit their forms to the nurse in the 9th Grade Annex
  • 10th-12th Grade students should submit their forms to the Main Office in the Main Building

Important

  • The approval process can take up to 3 weeks after Medical forms are submitted.  
  • Incomplete  forms cannot be reviewed
  • Students cannot participate or practice until approved

Important:The district requires that the ENTIRE packet be submitted .

When the physical forms (APE) have not been submitted or when the  date of the last physical is more than a year prior to the sport start date (see below). 

Physical status an be checked in Genesis under the Athletics tab.

When  the date of the last physical was less than a year ago, but more 90 days before the sport start date (see below)

When in doubt, submit this form.

ImPACT Testing

All athletes must successfully complete ImPACT testing before participating in a sport.  Valid ImPACT tests are good for two years. Athletes will be notified if an ImPACT test is needed.

  • ImPACT Testing will typically be conducted virtually (online)
  • The next testing date is June 18, 2024
  • If a valid ImPACT test is not on file, an email with the test link will be sent to both the parent and student
  • Students must be registered for all sports in Genesis no later than June 4th in order to receive the ImPACT registration link.
  • The test link will be emailed 24-48 hours prior to the  test session

Important: Full sports clearance can be seen on Genesis in the Athletics section. It is possible that it can say that you are cleared even if your ImPACT baseline will expire before the end of your season. It needs to be good until 11/1/2024 for most Fall sports, except football (12/10/2024).
