Note: These physicals are intended for those who do not have a medical home or cannot get a physical with their personal doctor.

Medical Forms

Deadline:  February 10, 2025 for Spring Sports

The forms below must be printed, completed, and submitted to the School Nurse.
  • Forms should be placed in an envelope which is then sealed
  • Write the Student's name, grade and sport clearly on the envelope
  • Freshmen should submit their forms to the nurse in the 9th Grade Annex
  • 10th-12th Grade students should submit their forms to the Main Building Office in the marked inbox.

Important

  • The approval process can take up to 3 weeks after medical forms are submitted.  
  • Incomplete forms cannot be reviewed
  • Students cannot participate or practice until approved

Important: The district requires that the ENTIRE packet be submitted.

When the physical forms (APE) have not been submitted or when the  date of the last physical is more than a year prior to the sport start date (see below). 

Physical status can be checked in Genesis under the Athletics tab.
