Medical Forms
Deadline:
November 4th for Winter Sports
The forms below must be printed, completed, and submitted to the School Nurse.
- Forms should be placed in an envelope which is then sealed
- Write the Student's name and sport clearly on the envelope
- Freshmen should submit their forms to the nurse in the 9th Grade Annex
- 10th-12th Grade students should submit their forms to the Main Building Office in the marked inbox.
Important
- The approval process can take up to 3 weeks after medical forms are submitted.
- Incomplete forms cannot be reviewed
- Students cannot participate or practice until approved
