Deadline:  June 5th for Fall Sports

The forms below must be printed, completed, and submitted to the School Nurse.
  • Forms should be placed in an envelope which is then sealed
  • Write the Student's name, grade and sport clearly on the envelope
  • Freshmen should submit their forms to the nurse in the 9th Grade Annex
  • 10th-12th Grade students should submit their forms directly to the Nurse's Office in the main building. 
  • Summer forms submission will be in the main office in the main building. Forms must be in a sealed envelope with Name, Grade, and Sport listed on the outside of the envelope. 

Important

  • The approval process can take up to 3 weeks after medical forms are submitted.  
  • Incomplete forms cannot be reviewed.
  • Students cannot participate or practice until approved.

Important:
  • Regular Doctor Visit: Submit only Medical Eligibility Form (pg. 5). 
  • If you do not have a medical home, please see the recommended locations below.

When the physical forms (APE) have not been submitted or when the  date of the last physical is more than a year prior to the sport start date (Check Sports page) 

Physical status can be checked in Genesis under the Athletics tab.
